Let’s Work Together

Let's Work Together logo

Let’s Work Together currently operates in Lichfield, Tamworth and East Staffordshire. This project aims to equip service managers and home visitors with the tools, training and skills they need to provide the ‘eyes and ears’ for partner organisations. It offers a wide range of support and signposting to local people to help them live healthy, safe and independent lives.

The first stage involves a risk assessment being conducted with the client. This is often undertaken in the client’s home, although it does not have to be, and assesses any dangers to the client’s health, wellbeing, and /or independence.

Once the risk assessment has been conducted, the home visitor or service manager can refer their client via an online referral process. The referral platform in Lichfield is operated by RAFT (Right Advice First Time). RAFT contact relevant organisations on the client’s behalf, giving details of the risk or risks and requesting assistance. The organisation then contacts the client directly.Photo of home care worker with a client

We welcome expressions of interest from organisations who would like to be involved in this multi-agency initiative, and provide all the training required to participate in the project.

We run a range of training sessions in Lichfield, Tamworth and East Staffordshire.  These focus on certain risk categories, exploring them in more depth.

These sessions are run with guest speakers from a range of organisations such as Staffordshire Police, Staffordshire Fire and Rescue, Community Safety Lichfield District Council and Tamworth Borough Council, England Illegal Money Lending Team, Staffordshire and Stoke-on-Trent Partnership (SSOTP) NHS, and South Staffs Local Pharmaceutical Committee (LPC).
