Let’s Work Together
Let’s Work Together currently operates in Lichfield and Tamworth. This project aims to equip service managers and home visitors with the tools, training and skills they need to provide the ‘eyes and ears’ for partner organisations. It offers a wide range of support and signposting to local people to help them live healthy, safe and independent lives.
The first stage involves a risk assessment being conducted with the client. This is often undertaken in the client’s home, although it does not have to be, and assesses any dangers to the client’s health, wellbeing, and /or independence.
Once the risk assessment has been conducted, the home visitor or service manager can refer their client via an online referral process. The referral platform in Lichfield is operated by RAFT (Right Advice First Time). RAFT contact relevant organisations on the client’s behalf, giving details of the risk or risks and requesting assistance. The organisation then contacts the client directly.
We welcome expressions of interest from organisations who would like to be involved in this multi-agency initiative, and provide all the training required to participate in the project.
There are a number of training options available. The e-learning module allows you to learn at your own pace. It guides you through the whole process including risk assessment and referral, and checks understanding via a series of questions. As you work through the modules you can download key documents regarding risk assessment, signposting and referral. This allows you to compile a reference manual which you can use whilst with your client.
