About the Role:
St Giles Hospice is looking for an experienced Social Prescriber, based at our Supportive Care centre in Holly Road. This role provides the opportunity to combine being part of East Staffordshire’s Social Prescribing Team with the co-ordination and development of Uttoxeter Cares, a collaborative community development project between St Giles Hospice, Katharine House Hospice and The Hermitage Charitable Trust.
You will be part of a team providing a social prescribing service to individuals experiencing a range of sometimes complex social circumstances, using agreed, person-centred based principles, building trusting relationships focusing on ‘what matters to me’ and providing personalised support. You will help people to identify their health and wellbeing needs.
You will also be responsible for overseeing the operational management of Uttoxeter Cares and work alongside our partners and local communities to support the development of a Compassionate Community in Uttoxeter.
We are looking for someone with good communication and interpersonal skills, who works effectively as part of a team and independently. You will need to be well organised, be able to get along with people from all backgrounds and communities and motivate others. It is essential you are comfortable talking about death and dying, be approachable, empathic and compassionate.

Having a CQC rating of outstanding, St Giles Hospice is a registered charity providing high-quality medical and nursing care for local people living with cancer and other serious illnesses, as well as offering support for their families and helpers. Care is offered in a variety of ways according to the individual’s needs and are offered out in the community as well as from 3 sites, an Inpatient Unit at Whittington, Day Unit Facilities at Sutton Coldfield and Uttoxeter.
We receive funding from the NHS but primarily we rely on fundraising activities and legacies, together with trading activities, 21 Shops, Lottery and Other Trading to support our work. We employ 300 people across a broad range of roles; from direct patient care to fundraising and administration as well as support from 1000 volunteers to help us provide and support our services.
