Primary Care Navigator

£19,597.50 per annum (Full Time 37.5hrs)

Do you have a passion for working with older people and for improving health outcomes? As a Primary Care Navigator (PCN) you will deliver a service in the community working remotely across Burton upon Trent.

The aim of your role is to enable people to keep healthy, independent and safe in their own home. Ideal candidates will have experience of working in health and social care. The PCN must have access to a vehicle in order to cover areas in East Staffordshire and surrounds.

Would you like to become a qualified Community Money Adviser and help people in Stoke overcome debt issues and the cycle of despair it brings? If you can spare a day or two each week to volunteer with Money Matters, we will pay for your qualification up to level two, plus we will cover all volunteer expenses.

An opportunity has arisen for an experienced and enthusiastic finance professional to work with a Christian charity, Saltbox, based in Stoke-on-Trent. Saltbox undertakes infrastructure support for local churches and faith based organisations whilst also delivering a number of projects including: Restart – the housing and rehabilitation of ex-offenders; CareLink – a befriending scheme for older, vulnerable people; and Money Matters – our debt advice service.

JOB DESCRIPTION:

Signposts (Stafford) Limited - Centre Manager

Directors are seeking applications for the post of Centre Manager, starting in February 2018. The post will be permanent, part-time, 4 days a week paid at the rate of £22,350 pro rata.
